Advanced Training Participant Agreement

 

By registering for this program, I confirm that I have reviewed and agree to the following:

 

  1. Eligibility & Professional Responsibility: I affirm that I meet the eligibility requirements for this training, including appropriate licensure or graduate-level status as outlined in the course description. I understand that IRIS Training Collective does not provide clinical supervision, and participation in this program does not create a supervisory relationship. I remain fully responsible for my clinical decisions, practices, and supervision in accordance with my licensing board and organizational guidelines.
  2. Attendance & Continuing Education Credit: I understand this is a live, interactive training via Zoom. Cameras must remain on during all sessions. Missing more than 15 minutes may result in no CE credit and no refund unless prior arrangements are made. Full attendance is required to receive CE credit; partial credit will not be awarded.
  3. Refund & Cancellation Policy: Refund requests made at least 24 hours prior to the event will receive a full refund. Requests made with less than 24 hours’ notice or for no-shows are non-refundable. If IRIS cancels, a full refund or store credit will be issued.
  4. Course Evaluation: I understand that I must complete the course evaluation in my account to receive my CE certificate.
  5. Confidentiality & Professional Conduct: I agree to maintain confidentiality regarding any personal or clinical information shared during the training. I will not share identifiable client information in group discussions or chat. I will engage respectfully and adhere to professional ethics at all times.
  1. Accessibility & Accommodations:  I have been informed that accommodations under the Americans with Disabilities Act are available upon request by contacting [email protected] at least two weeks prior to the program. 
  2. Privacy Policy: I have reviewed IRIS Training Collective’s Privacy Policy and understand how my information will be used and stored.
  1. Grievances: IRIS is committed to a respectful, inclusive learning environment. Concerns or grievances can be submitted in writing to [email protected]. IRIS will review and respond within 5 business days.
